I have had my braces on for nearly five years i have decay on my teeth

Q. I have had my braces on for nearly five years. I have notice decay on my teeth. I know I’ve probably ruined my teeth. But it has been five years. Is there anything I can do to whiten my teeth? Or are they going to stay like that? Please Help! Im nineteen now.

A. Hi

I would recommend you book an appointment to diagnos the areas of decay and treat them as soon as possible. Of course by leaving decay there is eventual risk of loosing the teeth.

These decayed areas will be able to be treated by white fillings.

I had root canal treatment done about 6 months ago

Q. I had root canal treatment done about 6 months ago. My tooth started hurting about 4 days ago and now I am in a lot of pain. When I bite down on the tooth it really hurts. If I tap the top of the tooth it hurts and feels sensitive. I also have a tingly feeling and pain in my jaw. What is the problem? Many thanks

A. The most common causes are a failed root filling with an abcess forming or root fracture. It needs urgent treatment either way. If you need advice urgently we will be around tuesday am 0121 643 0610.

ive had braces for a year and had them taken off the teeth have now moved

Q. i had braces for a year when i was 17 i had them taken off when i was 18 and my teeth were perfect. but they have moved alot my upper teeth over lap the bottom but they also point upwards. when i noticed this problem i contacted my old othadontist and he wouldnt see me because im over 18. but iv started getting problems with tooth ache now i can actually feel them moving and its causing alot of pain, all the area surrounding my mouth also aches due to me not being able to close my mouth because the gap is so big. this is really getting me down and iv become really concious about myself! as i am a student i dont have the funds to go private so would i be able to get the treatment needed on nhs?

A. Unfortunately NHS orthodontic treatment is only available to under 18s. However, no treatment which requires orthodontic correction should be causing an acute toothache, and i think you may have something else which is causing the problem.

The first thing you need to do is get yourself to see a dentist and once he is happy that no problem exists discuss options for the possible treatment to correct the relapsing orthodontics.

my teeth color from my childhood has changed

Q. Dear Dr, I have problem with my teeth color, form my childhod my teeth color is changed, i treated many times by using whitening bleach, but not changed, Dr advised me to crown my all teeth,so i need ur advice. thanks waiting for quick respose

A. Don’t crown your teeth. I’m sure the colour can be changed using veneers. Unfortunately whitening does not get rid of all types of stains. I would advise you to go and see a a cosmetic dentist and enquire about veneers

i have internal stains in my teeth and my doctor had removed the top surface of teeth

Q. hi.. i have internal stains in my teeth and my doctor had removed the top surface of teeth in order to remove stains but they r still appearing and becoming yellow in colour and for one of my front tooth is placed with crown and these things together are looking odd.. so please tel me wat would be the best opinion for me to remove internal stains so that i have perfect match with my crown… i want to know whether laser treatment would help and is it safe bec my top surface of my teeth r removed…please suggest me the best opition..please…….

A. hi many thanks for your enquiry, the internal stains may improve with laser whitening but unfortunately this cant be guaranteed. my advice would be to maybe consider having the front teeth in your smile veneered and the crown replaced with the same porcelain that the veneers are made of. this would give you a perfect colour match and remove the internal staining for good. hope this is helpful.

I am in need of dental implants

Q. I am in need of dental implants and understand that I will need assesing . Could you send me a rough guide on what I would expect to pay for each implant.. I need about 5 in total i think.. Ps I work at the royal London hopsital myself

A. Hi  The assessment process involves a thorough diagnosis and the formulation of a detailed treatment plan to encompass the whole surgical and restorative treatment involved with dental implants. You may also require a CT scan. This process normally costs around £300 after which, as  a general guide, the implants will cost around £2000 per implant complete with a final crown.

Let me know if I can be of further assistance

i had a accident which left me with half of one of my upper front teeth

Q. Hi, Around 20 years ago, i had a accident which left me with half of one of my upper front teeth, around this time they managed to build the touch back up, however since then it has moved forward and now hangs in front of my other teeth, I find this very uncomfortable and i find it can effect my speech. I would really like to finally get this fixed, can you offer me the best cause of action to help me with this. Regards

A. I would need an Xray of the bone around the tooth to help make a diagnosis. There are a lot of good options to reallign teeth or in some cases replace the tooth.
Because the best choice depends on a clinical assement I recommend getting a couple of opinions before starting.
We offer a free initial assesment to provide you with some of the better choices available to get the right outcome for you.

my problem is that i dont like my teeth

Q. hi i just wanted to know what sort of treatment do i need my problem is that i dnt like my teeth i had braces when i was 17 and i took them of when i was 19 i am now 20 and im still ot happy with them my orthodontics said that i didnt need them anymore. i jus want me teeth to go more back i was wondering can you tell me how much the six month smile costs or what else could i use?

A.

teeth can only be moved a certain amount so they not be able to be moved anymore.  Orthodontic options may be an option or you may need a combined orthodontic and cosmetic approach.  Prices vary depending on what you need I recommend finding a good cosmetic dentist and discussing the options that are available to you.

what type of implant and precision attachments aid in denture retention

Q. i would be grateful if you could help me. i would like to know what type of implant and precision attachments aid in denture retention?

A.

Hi

There are many different options. Of course the options depend on -  is it for the upper or lower, amount of bone you have, type of bite you have, etc.

The possible options are
1. locators
2. magnets
3. studs
4. bars.

the following web page provides different videos which explain the different options in more detail - http://www.dental-suite.co.uk/dental_implants.htm
(just click on the video link on this page)

I personally tend to use locators or bars to provide denture retention.

Ive had yellowish teeth since i was young due to antibiotic medicines i took

Q. Hi, I’m having yellowish teeth since i was young due to antibiotic medicines i took. I tried whitening toothpaste & treatment but seems can;t work. I’ve read about veneer treatment. is it the only option i have? how about laser treatment? what is the pros & cons?

A. Tetracycline staining is all the way through the tooth.  Long term bleaching can substantially improve things.  Bleaching can be a combination of home bleaching and “laser” bleaching.  In my practice we only carry out home tray bleaching with a surgery visit of slightly stronger bleach placed in the tray - I have my doubts as to whether laser bleaching is really effective.

However veneers will provide you with a good end result immediately.  The down side for veneers is that a surface layer of tooth needs to be removed ( 0.5 to 0.7 mm especially if the staining is dark) . If the teeth are in good alignment there usually are minimal problems with this technique - years down the line you will need them replaced with more veneers.  If the teeth are in poor alignment there is a chance that root treatment may be required if the neves of the teeth are damaged.  In this instance I would advise some form of brace work prior to veneers to minimise the damage done to the teeth.

I have treated many people this way very successfully but the important thing is that you realise that once your teeth are veneered there is no going back.

My front four top teeth are in need of crowns

Q. Hello there, I’ve seen your contributions to dentist’s blog at : http://www.cosmeticdentistryguide.co.uk/blog/hi-i-was-looking-for-some-advice/ My front four top teeth are in need of crowns, I think. Currently they are heavily filled, discoloured and I hate them. Could you please advise how do I go about choosing a good dentist? Thank you in advance for your response.

A. The best thing to do is to look for cosmetic and restorative dentists on the internet and then go and have consultations with them. You need to ask about their training and experience. They should hae training in Ocllusion which is a study of how the teeth, jaws and muscles function together. Definately look at their portfolio. If they don’t have one then don’t go to that dentist. You will be able to judge yourself by the end results of treatment that they show you

my Dentist told me that I am loosing one of my front tooth

Q. I am 42 yrs old and I recently went to have get cleaning done and my Dentist told me that I am loosing one of my front tooth, he said it cannot be saved it will eventually just fall out. I also have a little abrasion on the inside which feels very funny but is does not hurt.I keep putting my tongue on it. I am scared as I need to know what can be done at this point in time.

A. Well, unfortunately situations where teeth become mobile do occur.  Eventually they are lost and a replacement option is required.

The three options for replacing a missing a tooth are 1. a denture, 2. a bridge 3. a dental implant.
The option for replacement depends on the cost and the surrounding teeth.

You should speak to your dentist and go through the options so that you have a plan in place for when the tooth is lost.

I am 26 and my mouth is a mess

Q. I am 26 and my mouth is a mess. I was a meth addict for 10 years and it destroyed my teeth. Over half of my teeth are broken and down to the gum line exposing the root. There’s not one tooth in my mouth intact. I am in so much pain everyday and have been for 2-3 years. I am so ashamed i don’t have any confidence and am constantly reminding myself when around people even my husband to turn my head away or cover my mouth so they can’t see my teeth. After the birth of my 4th child i was diagnosed with CHF i have to take 5 different meds daily for life. My dr said me being so young the meds could strengthen my heart but unless i get these teeth out of my mouth i can’t get the full benefit of my meds. I also believe once a dentist sees my mouth he won’t want to bother because it’s going to be so much work to remove them i need to get this done ASAP my husbands insurance doesn’t kick in for 30 days Do u think an oral surgeon will go ahead and start on my mouth and accept payments or do i need to wait until insurance kicks in

A. A general dentist should be able to manage the dental aspects of your treatment. You may be exempt from dental charges try NHS direct for links. At this stage it might be better to clear out the hopeless teeth do some simple fillings on the NHS. IF you find out that you are not exempt the fees would be £198 at an NHS dentist. As you are in pain I recommend seeing a dentist now and find out what treatment you need / estimated costs ( if any)

my problem is overcrowed bottom teeth

Q. Hi, my problem is overcrowed bottom teeth, slight overbite from top teeth(sucking thumb child), uneven bite (left side teeth just about touch compared to right), narrow(ish) bite, 3 venners and crown (4 front top teeth due to broken teeth). The thing is I’m 30 in a sales role, any chance Invisalign will work for me? Its as much for comestic reasons (fear it will look worse as i age) as it is for hygiene as my teeth never feel really clean even after flossing.

A. i think you may need a number of different treatments.  Invisalign could straighten and re align your teeth and improve your bite, after this you may need some cosmetic to replace old restorations and create a new smile.

I have triangular gaps between my teeth owing to gum recession

Q. I have triangular gaps between my teeth owing to gum recession. My gums are stable and I have no loose teeth. Can I have composite bonding to fill the gaps and if so, could you recommend a highly skilled dentist who I could go to for this procedure? Many thanks

A. Hi

It is possible, but you would require an examination to see if could be done.

To recommend you a dentist, i would need to know the area you are looking for one.

I am 55 years of age and have just had a bridge removed

Q. I am 55 years of age and have just had a bridge removed and have been fitted with a plate with three false dentures at the front and four at the side. I am intererested in implants, and am wondering at the cost of implants and whether it is feasable for me?

A. Implants can be placed in many situations.  The key is to the amount of jaw bone available to place them in .  If insufficient grafting techniques can be used to increase the volume of bone available.  You would need a proper assessment before any costscan be given to you.

I had a route canal but did not get the crown yet

Q. I had a route canal but did not get the crown yet. The filling just came out. What can I do until I get into the dentist? The office is closed until Jan 5, 09.

A. You can go to a pharmacy and get some temporary dental materials to cover the tooth until you can see your dentist.  Emergency dentists are available.

i have recently had crowns caps not sure which over my teeth

Q. i have recently had crowns/caps (not sure which) over my teeth. i still have little “stubs” underneath the crowns but not much as they were filed down. unfortunately my new front teeth stick out slightly and was wondering if braces were an option and how much would they cost on the nhs? thanks

A. Their are parameters that decide whether a patient has enough of a problem to warrent braces on the NHS.  You would need to be assessed by an NHS dentist.
