Q. I am 69yrs old and in good health. I have had to have two root anals done on my #15 molar. There is still an infection at the root area. My endodontist feel that the root is short and the crown is small since it has been capped. The endodontist thinks I should just remove the tooth and not bother with an implant since the jaw in that area has very little bone to set in an implant. My regular dentist removed the cap before I saw the endodontist, leaving only a small tooth crown. I have had three other implants that have worked very successfully. Two of the implants have been in for fourteen years.

A. The available volume & quality bone will affect the chance of a succesfull outcome for implant treatments. each region of the mouth is different even for a given patient
If you have specific clinical questions relating to your own best course of action I would recommend a second or third opinion which would require a clinical assesment

A. The ways of treating your problem are listed below and may be a combination of them:
1. alter the position of you jaw bone by surgery
2.  alter the positiojn of your teeth with orthodontics/ gum repositioning with veneers or a combination of both
3.  a new technique is limit upper lip movement  but I know very little about this procedure

I have had a pretty serious tooth grinding problem

I have had a pretty serious tooth grinding problem for some years now which is starting to affect the size of my teeth - I am noticing that some of them are shortening and the backs of them are very worn down and also they don’t seem to fit together properly anymore. Also I have the usual headaches and pain in my facial muscles which are also often swollen. I have had treatment for grinding for quite a few years and I have a mouthguard tanner appliance but I am concerned that my teeth may start to disintegrate. Is the best thing to thing about veneers now or is there any other form of mouthguard which stops the teeth actually touching? Is there any proven method for minimising bruxism? Any advice gratefully received… Many thanks

A. There is no known way orf stopping bruxism.  The best thing that can be done is to reduce the possibility of you damaging your back and front teeth by using restorations to reorganise how your teeth work and to regain the vertical dimension that you have probably lost.  This treatment usually requires a full arch ( uper or lower) and sometimes both arches to be crowned, overlayed or veneered.  After this a night time appliance is usually required to protect the final restorations.  As a general rule no treatment should be completed until your muscle pain has been relieved - probably with a new appliance to reequiibrate your occlusion prior to commencement of treatment.

A. Thank you for your question.
 You desperately need to find a dentist. A private dentist will be able to give you all the options available for your teeth. It sounds like you need a lot of treatment and if you fear injections you may be able to have sedation. The breath problem could be due to gum disease which can also be treated and rectified. The longer you wait the worse it will get. There are many dentists that will carry out treatment at your pace and explain everything to you. Unfortunately there is no way to avoid the drilll. The best thing to do is to find a dentist that you feel comfortable with and talk to them about your fears so that they can be addressed.

A. You may need to be treated by a specialist periodontist ( gum specialist) .  Even so the gum health may be too severe for treatment.  Implants should not be placed if there is active gum disease.  Once controlled they may be a possibility.

A. You would need to undergo full brace treatment for this problem. This means fixed braces for up to 18 months. The gaps will close properly and woth permanent reatention they should stay closed. Now is the time for you to have braces as I am sure many of the people you go to school with also have them. If you have it done properly now you will not have to worry about it when you are older.
Ask you dentist to refer you to an orthodontist so that you can get all of your treatment options

The swelling was preent all these weeks after the big swelling went down. Can anibiotics work on an infection around the implants. Thank you very much!

A. strong antibiotics can sometimes remove the infection, however it sounds like your implants are failing.  Your dentist will review then and test them for integration ( fusing to your bone).  Sometimes new bone can be grafted around your implants and this can save them.

A. There are 2 types of sedation,
the first is relative sedation which is carried out with Nitrous oxide, ‘laughing gas’
There are some dentistal clinicas that still offer this but very few.
The other sedation method is IV, intravenous sedation. A consultant anaesthetist carries out this treatment at a dental practice or in hospital. The medications are applied through a vein in the arm or hand.
If sedation is used for treatment you have take the whole day and usually the next off work because of the after effects of the sedation. If you find a dentist that offers the treatment they will give you medical forms that need to be filled out by the doctor to make sure of any medical issues that may hinder this type of treatment

I’ve just had tooth removed today and a temporary

Q. I’ve just had tooth removed today and a temporary denture fitted. The tooth looks great as the old one was damaged but the plate feels weird in the roof of my mouth and is making me gag and affecting my speech quite badly. Is this normal? How long will it be before I can speak properly? And when will I feel like eating again? My apetite has completely gone due to this piece of plastic in my mouth.

A. Your muscles need to adapt to this foreign object object in youtr mouth, with time you will find you can speak and eat well.  Your denture will probably need replacing or relining ( coating on the underside of the denture to improve the fit) after a couple of months as your gums shrink after an extraction.  You can always look at eliminating the denture by having a dental implant or bridge.

I have had infections after poor root canal treatment

Q. Hi. I have been having consultations with a dentist re having a problem root (poor root canal treatment many years ago, since infected on the side, now apparently perforated) removed and replaced with an implant. However, I am a bit concerned and would appreciate some advice. The dentist (private) advertises implants as a service, but despite my requests, after two one hour consultations has been unable to show me photos of a completed implant (apparently they were all held on a computer and got lost) and keeps evading my questions on how many implants they have done by just telling me that he devotes two days a week to implant work of some description (the practice is based in a small town in a not overly affluent part of the country). The only ‘evidence’ available to me so far is the mobile phone no. of a patient who has recently had the implant placed (the crown is not in situ yet). I need to get the tooth removed soon as I can feel the infection starting to return, but am concerned as to whether I would be making the right decision in allowing this dentist to remove the tooth. Could you give me any advice please?

A. You are asking your dentist valid questions, if you are not happy with the responses it is worth getting a second opinion. How can i close a front teeth gap

Q. i have a gap between my 2 front teeth, and i would like for the gap to be closed to make my appearance better. i wore a brace when i was 13 years old as my right front tooth was quite crooked, although i wore it for 12months, the tooth wasnt completely straightened. i have always been conscious of the gap and wondered how i go about getting it closed.

A. You can either have the gap closed with white fillings or with porcelain veneers. If the gap is small you may get away with having a couple of veneers placed or a couple of white fillings. If the gap is large you may need more treatment to avoid ending up with 2 huge teeth at the front which can look as bad as the gap. You could of course have braces again but you know that this takes time.

The best thing to do is go to a cosmetic/ restorative dentist and you will be able to discuss all the options

Q. I gave up smoking 2 years ago,and in that time my lower gums have receeded, to the point of 2 wobbly teeth. I visit my dentist regularly for check ups, and he says i have gum disease and there is nothing i can do about it. I brush, floss and use mouth wash daily. If i need any treatment, i pay to go private to be heavily sedated, as i am an extremely nervous patient (my Mum takes my kids! and i can’t even watch the videos on this site). My question is: i would like to have veneers fited as my teeth are truly awful, but my problem is my fear. Is there anywhere that can sedate me, put me to sleep to have this done. Also can i treat my gum disease, i don’t want to loose my teeth (i lost loads whilst pregnant with my son, as my dentist wouldn’t treat me. I had a calcium problem when pregnant, resulting in the loss. I just want a beautiful smile.

A. Your gum disease usually can be treated but due to the recurrent nature of gum disease it is unlikely that you can be heavily sedated for treatment as it will require numerous appointments to get your disease under control.  From then on you would need regular ( probably 3 months) visits with a hygienist to keep the status quo.  Your gum disease should be controlled before you embark on cosmetic treatment but you can be sedated for the veneer preparation and fitting ther veneers.
